Bulbous Tip Rhinoplasty: Open vs. Closed Technique

Furthermore there are a limited number of grafts you can place and usually you cannot secure them other than the pocket you make for them so they can shift during healing.

In my experience, patients who may be initially happy with their tip results after endonasal rhinoplasty, do not infrequently develop issues as the skin soft tissue envelope contracts, and by 1-2 years after surgery the tip can become pinched or distorted. Many are underwhelmed with the degree of refinement they achieve, especially in boxy tips, while others can feel the nose looks pinched when suture techniques are placed too tight.
